The Children’s Hearing Institute independently evaluates applications from US healthcare providers for the following implantable technology:
- Cochlear™ Nucleus® Systems
- Cochlear™ Osia® System
- Cochlear™ Baha® Systems

*US healthcare providers will be asked to complete an eligibility checklist to verify that certain requirements are met. If so, you will be invited to complete an Implantable Device Donation Application.
**Please note: receiving the application does not indicate that a device has been approved. All applications must be completed and will be reviewed by our organization.
